If you are on a MEDICARE ADVANTAGE PLAN (this takes the place of your traditional medicare), please call the 800 number on the back of your card. These change yearly and are very picky about “your network”. i.e.: Aetna Medicare Advantage PPO is not the same as Aetna PPO plan. They do not have the same networks. BCBS HMO Advantage plan is not the same as BCBS plans.
Medcost, PHCS, and some out of state insurances are UMBRELLA insurances that cover hundreds and hundreds of small plans and will have separate benefits for each plan. Call the 800 number on the back of your card for specifics. They should be able to help you.
Every policy differs in which services they will compensate the patient for so make sure you check the website listed on the back of your insurance card or the use the 800/866 number to verify your benefits.
